The report was made to the LPN on [REDACTED] at the beginning of 2nd shift @ [REDACTED] when [REDACTED] was also starting her work for that shift. She stated she felt she hurt her back while giving a resident a shower the previous day and it was still causing her pain this following day of work. She was advised to complete a work-injury report.
