[REDACTED] stated he got on his forklift to strap a pallet and as he climbed on the deck the board flipped over causing him to fall backwards and lose his grip. He stated he fell on his forks and then onto the ground. He said he slowly sat up and assessed the damage and pain after a few minutes and then he reported it to me.
