Stated that he wasn 't feeling the greatest [REDACTED]. Came into work and was breaking freight on the dock around 25 door and around [REDACTED] felt a headache nauseous and dizzy. Stated that at around [REDACTED] he wasn 't sure if he lost conciousness for a split second or was about to pass out but came to me and stated that he needed to go get medical attention. I took him to Genesis emergency care in silvis.
