What happened
[REDACTED] had a delivery to customer. Walked through walk in door for dock proceeded to unload two pallets. Proceeded to walk through the building to have someone sign her delivery receipt. Saw light coming from an open area where she seen people working walked to another doorway walked through that doorway and stepped in a 2-foot hole where a metal grate was removed. Door was unmarked and not locked and the grate wasn 't covering the hole.
