cases had been getting stuck at corner of the conveyor due to a missing belt some cases were pushed through by next case coming down but TM and lead were also clearing cases when jams occurred one case was being pushed by the next case coming down and fell off the conveyor sticking TM in arm who was on the ground floor case just reached TM and TM stated that TM was not significantly injured
