What happened
At the start of our shift [REDACTED] was setting his station up and picked up rolled up the mat to put it away. When he went to put the mat on the mat rack it unrolled when it hit the floor causing dust to blow up in his face. He had a foreign object enter his left eye. He was wearing sealed eyewear gloves steel toes and ear plugs.
