What happened
Employee was covering a break on the bull ladle. He was pouring iron into DISA 1 autopour when some slag plugged up the spout. He wheeled it back and moved towards the slag area. He picked up a slag ron and positioned the spour toward the stairs. He used the slag rod to push the slag back into the molten and started moving the ladle towards DISA 2. He moved to lay the slag rod back against the beam and the bull ladle wheel rotated back towards the beam pinning his arm against it.
