Before the incident
Removing scrap off the top of the die with his left hand.
What happened
First employee was trying to remove scrap from the top of the die. Second employee was trying to adjust the position of the die without space bar in place. Second employee engaged the bar selector switch which caused the die ram to land into its bottom position. The first employees hand was caught when the die landed in its bottom position.
