Employee stated he was looking for a material leak in the hose room and ascended up the fixed industrial ladder to the catwalk and as he was descending down the ladder he was facing away from he rungs and did not have 3-points of contact. His foot slipped and he fell to the ground causing an injury to the left ankle.
