What happened
there was a small scissor lift blocking the fixed ladder. The employee ducked under the scissor lift and began to climb the ladder. the employee adjusted his footing and turned his foot sideways on the ladder rung. the employee lost his grip on the ladder and fell from fixed ladder hitting the scissor lift guard rails and fell to the concrete floor below.
