What happened
The employee was using a mallet to hit the side of the dosing hopper in order to unstick the product that had built up inside of it. The side of the hopper has an access panel that is held in place by 6 bolts. The employee was holding the mallet and standing beside the equipment in such a way that when he swung the mallet his hand made contact with the bolts.
