Before the incident
Working in the turbo sub job lifting turbos and I started feeling pain in my shoulder that 's when I started going to HYP
What happened
EE to medical for FROI with supervisor [REDACTED]. Per EMMS statement: Lifting the turbos over time has started making my (right) shoulder hurt and shoot pain down my arm. States discomfort started [REDACTED] and started seeing ESI [REDACTED]. Deferred to Medical by ESI on [REDACTED]. EE is [REDACTED] male Nissan EE for 10 years. EE is working in [REDACTED] and attributes injury to turbo sub job. EE reports pain as 1on 0-10 scale and is sore and disturbs sleep. States has pain when raising arm above shoulder height. Pain radiates to down upper arm. Pain did not improve with ESI treatment with heat ice massage. Pain worse with above shoulder motions and working turbo sub job. Allergic to Zyrted. Prior WC injury [REDACTED] head laceration. Denies job outside of Nissan. Lists no hobbies.
