Before the incident
STF [REDACTED] RN was assisting with patient from [REDACTED] 14 on __R[REDACTED]ACT[REDACTED]__ 25 at [REDACTED] when they were struck in the face by patient verbally abusive and scratched. EE reports that the patient was threatening to kill everyone in the unit and a code green was called. Patient threw items at coworker and then returned to room. EE was standing at the doorway to [REDACTED] 14 when patient got back in bed. EE instructed staff to get restraints and patient jumped up on bed EE told patient to sit down patient charged at EE and punched EE in left side of face. Patient them scratched below [R[REDACTED]ACT[REDACTED]] bottom lip not breaking skin. Patient then shoved at EE pushing their glasses into their face.
