The injury occurred as team member was checking the Oring placement with his left index finger on the ramair die on the west side inside of a 2500t press As the employee was checking the Oring his right hand was on the control switch and engaged the button forward This caused the left index finger to be crushed within the fixture This resulted in an amputation above the first knuckle of the left index finger
