At [REDACTED] on this date [REDACTED] responded to [REDACTED] for an employee injury report. Upon arrival to said room Housekeeping Mgr. [REDACTED] was already on location preparing an icepack. This Officer observed housekeeper [REDACTED] lying on the floor in pain. When asked what happened [REDACTED] stated that as she was walking over to the door to receive the Linen that houseman [REDACTED] had brought up she tripped and fell on the untucked sheet that was hanging off the bed. [REDACTED] further stated that as a result of the fall to the floor she is now experiencing a lot of pain in her right shoulder and right knee which she [REDACTED] prior had surgery on. At [REDACTED] 's request [REDACTED] was called for EMS. EMS was initially called at [REDACTED]. Due to no response a second call was placed at [REDACTED] and no ETA was given. At [REDACTED] EMS from [REDACTED] arrived and was escorted to [REDACTED] by H O Leverpool. [REDACTED] was placed on the gerny and was transported to Mt S
