Before the incident
All the storeroom attendants were checking the pallets of produce against the invoices that were staged at the loading dock before transferring to food storeroom.
What happened
EE was checking produce products at the loading dock with the coworkers. When coworker driving was electric pallet to bring a produce pallet to the food storeroom a piece of shrink wrap was still attached to the pallet. EE 's feet were under the shrink wrap piece that pulled him backwards. He tried to hold onto the pallet jack handle to break the fall with force pulling his legs he fell on his lower back.
