What happened

DNO2 On [REDACTED] at approximately [REDACTED] a DA for SMCO reported an injury with an initial pain level of a 9 10 in their Left Ankle. The DA was at Under [REDACTED] Loadout pad and the DA was pulling carts out warehouse. [REDACTED] from carts fell on DA left foot. After the investigation the root cause was determined to be [REDACTED] fell on DA foot.
