What happened
Employee was pushing the mold from the pouring line onto the transfer table. The rollers got stuck causing the mold to come to an abrupt stop and the employees momentum pushed them forward. The employee fell forward and onto the mold where their left hand came in contact with the molten metal left on the mold and the cup.
