What happened

placement of a mass timber beam on the third floor of the project site. [REDACTED] was serving as the bellman for the crane and was positioned near the edge of the structure to assist with landing the beam onto a floating column. As the beam was flown into position [REDACTED] recognized that it was too low to clear one of the braces securing the column and called for a slight bump up. Crane operator responded but during the adjustment the north end of the beam caught a brace on the floating column. Although the column had been temporarily braced in multiple spots using 2x4s with turnbuckles the force of the beam contacting the brace caused one to snap resulting in the column shifting violently and ultimately falling from its position and off the building. As the column fell it caught [REDACTED] ' fall protection harness pulling him off the edge of the structure. His fall was arrested by the fall protection system; however during the swing he collided with a nearby shoring tower causing
