Before the incident
Agency employee was in the process of completing his pick list while operating a reach truck.
What happened
Agency Employee was performing picking tasks. While attempting to retrieve a concrete table from an aisle he encountered a pole rack that had been improperly placed on top of the vendor pallet containing the table. Agency Employee tried to remove the pole rack but was unable to do so. In an effort to access the product he began pulling the table from its location which caused the pole rack to fall and break the pallet. As a result the table became unstable and fell onto the top of his left foot.
