What happened
Employee was attempting to clear backed up product on beller table. Table loaded a second piece of PVC pipe onto flipper that is only designed for one piece of pipe. The flipper raised into air with both pieces of pipe. As employee approached the end of crate second piece of pipe fell and bounced up causing a laceration to employee 's left cheek.
