Before the incident
Employee was getting product from the freezer and slipped and fell.
What happened
Employee States: Employee states I was in the Freezer retrieving product and brought with me a black 3 tiered production cart. I loaded the items onto the cart and turned to leave the freezer. I was pushing the black cart when I unknowingly stepped on a build up of ice and slipped and fell. When I slipped I fell backwards and hit the back of my head on the ground: when I realized what happened I noticed a throbbing pain in my head and when I stood up I felt very shaky. I did not see the patch of ice on the ground until I had already slipped and fell on it.
