Before the incident
filling bottled water (4pks) in aisle 19B his foot was placed in the opening of the pallet where the forks go on the opposite side of the hand jack.He lifted the water to place it on the shelf and he went to turn towards the shelf but his foot was caught inside the opening so instead of turning his whole body only his back twisted.
