Before the incident
Loading racks onto paint bread cart.
What happened
As she placed the rack onto the cart she then tried to push the rack from the bottom of the rack to adjust the racks. This caused the rack to dislodge from the top of the rack. The rack then became unhooked fell forward and the middle of the rack hit the employee toward the top of her forehead hairline causing a laceration and mild concussion.
