Before the incident
on [REDACTED] at [REDACTED] the Line 7 operator was pushing up the line using a fork truck The operator was working overtime and is not technically assigned to the department (maintenance trainee) Another truck operator was driving by on a VGV and observed that the trucks were not staying aligned against the rail The operator stopped dismounted the VGV and attempted to manually push the tread spool truck back against the rail During this attempt his leg became caught between the tread spool truck and the VGV It was later observed that one of the trucks was facing the opposite direction
