What happened
EE was climbing up an orange folding ladder that was not opened with the spreaders engaged ladder was leaning on the equipment (Going against company training policies) EE was going to put a lanyard back up that they use to hold open the scrap chute. Upon climbing the ladder the ladder slipped out from under the EE. EE was up on the 2nd rung when it slipped out.
