What happened
At approximately [REDACTED] on [REDACTED] the employee was working on line 1 at the blend tank. The employee pushed the lid up with both hands expecting it to latch in the upright position. The employee moved their right hand to the rim and then let go of the lid with their left hand and the lid came down onto the employee 's right middle finger. The employee immediately pulled their hand back and went for help.
