Before the incident
[REDACTED] was a trainee on a front load truck ([REDACTED]). Trash had fallen onto the canopy while dumping an overloaded container. [REDACTED] used his reach pole to remove some of the trash from the top of the truck. The reach pole would not reach all of the trash so [REDACTED] climbed on top of the truck to remove the trash.
What happened

[REDACTED] used his reach pole to remove some of the trash from the top of the truck. The reach pole would not reach all of the trash so [REDACTED] climbed on top of the truck to remove the trash. As [REDACTED] stepped onto the door at the top of the truck the door slid causing [REDACTED] to slip and hit his left ankle on a metal bar that is on top of the truck.
