Before the incident
Performaing a die cast machine installation at a customer site. The employee used a boom lift to access the upper part of the die cast machine. The employee was connected to the boom lift by a fall arrest harness. The employee climbed down from the boom lift platform under the machine 's ejector plate.

The employee lost his footing and grabbed onto the boom lift and hoses connected to the upper clamping cylinder to prevent himself from falling.
