[REDACTED] states that she was at work assisting with a dressing change when the pt decided that she didnt want to be on her side anymore and started pushing on the side rail causing [REDACTED] to twisting her left side. [REDACTED] rates pain at 2 10 when sitting and 10 10 when she starts to stand and walking. States that when she gets going though shes ok. States that heat ice and stretching have helped. Takes IBU for pain. - [REDACTED] LPN
