Before the incident
Staff could not get out of the chair to avoid patient in timely manner when the patient suddenly approached staff unprovokedly. Patient also has been refused medication frequently.
What happened
The patient suddenly ran out of her room and started to scratch staff face and punch staff head unprovokly. Staff fell on the floor and the patient continued to punch and scratch the staff without stopping.
