What happened
[REDACTED] delivered her baby after [REDACTED]. she was gotten up to the bathroom 2hr pp with 2 nurses per protocol. pt ambulated WNL with x2 standby. was able to get to and use toilet and get cleaned up. after standing and putting on underwear and pad this RN observed that pt looked like she was going to pass out - called to second nurse [REDACTED] who was just outside the bathroom door. this RN was able to safely hold pt and lower to the floor. 2nd rn came into room. this RN was unable to move - back was locked in crouched position and in spasm. [REDACTED] helped to get pt to a more assistable position - myself and [REDACTED] both heard my body make a crunch and pop noise. I have been in pain since. OHS was unavailable at the time of the incident
