Employee was attempting to move with an overhead crane a bundle of tube from the 3rd (top) layer of a stack. As the top bundle was lifted from the stack the bundle stacked closest to the employee rolled off the stack and onto the employee 's foot.

When he lifted a load up in the air the load below was unbalanced and one of the bundles rolled off and landed on his foot.
