Before the incident
Employee was loading a [REDACTED] MDF skin from the scissorlift table onto the CNC table around [REDACTED] on [REDACTED] As EE was lifting skin with their hand to slide it onto CNC table the skin slipped from their hand and fell onto rail edge of machine While falling MDF skin caught EEs left hand and crushed the middle finger EE reported it to Lead who sent the EE to speak with EHS EHS recommended calling Medcorp and keeping the finger on ice while doing so EE was uncertain as to whether they wanted to call MedCore or not so EHS recommended icing for a duration of time and then seeing if the EE wanted to call After icing for 20 minutes away from the floor EE reported to EHS that they could not bend their second knuckle EHS and Supervisor recommended Medcore again EE refused and signed a medical refusal form (attached) on [REDACTED] ON [REDACTED] 25 Associate continued to complain of pain and Medcore was utilized and was directed to Concentra
