Before the incident
The associate had dismounted Powered Industrial Vehicle Tugger and was in between a rack and the tugger unhooking the rack to push lineside for parts delivery.
What happened
NXAL Associate was operating a powered industrial vehicle tugger with a rack behind. The NXAL operator dismounted and went in between the rack and the tugger to unhook the hitch joining the rack and tugger. The rack once unhooked could then be pushed to the manufacturing line for delivery of parts. A forklift operator from a different company rear ended the rack our NXAL associate was unhooking causing crush point pinning injury between the rack and tugger.

Injuries were sustained to Right and Left lower extremities below knees with Left ankle fracture and right posterior knee laceration along with multiple contusions and abrasions.

Rack pinned crushing injury.
Summary line
Associate crushed between rack and tugger by being struck by another forklift. This caused left leg trauma and the left ankle to break and right lower leg trauma with laceration behind knee.
