What happened

Employee was applying foam cleaner (Power Foam [REDACTED] [REDACTED]) to the wash bay for wash down. Once completed he was rolling up the hoses for the foam cleaner pump a quick connect for one of the hoses failed and the hose popped off. This caused the employee to be splashed with chemical on his face and eyes.
