Before the incident
Assisting the supervisor with removal of discharge chute.
What happened
While a supervisor and an employee were working on the discharge chutes of the [REDACTED] the supervisor was in the process of removing a section of the equipment. During this task the chute detached from its bracket unexpectedly. The supervisor unaware that the employee was standing in close proximity lost control of the chute when it released from the bracket. The chute struck the employee on the left side of the forehead resulting in a laceration swelling and lightheadedness.
