What happened
[REDACTED] [REDACTED] went with orientee to assist [REDACTED] was throwing markers at staff and bang head wanted to move to [REDACTED] 3 (seclusion room) for pt safety. While pt was in seclusion pt put mat up to occlude window EE went in to go hands on. [REDACTED] was spitting at everyone while EEs were applying restraints. Restraints needed to be adjusted went hands on again. EE was holding pts leg pt saying Im going to bite my tongue off Im going to bang my head to make it bleed. [REDACTED] continued to spit on staff. EE was wearing a face shield (surgical mask with attached face shield) spit landed on head EE unsure if blood was in mouth or in spit or if any got in her eye. EE wiped off head washed hands and wiped face with paper towel. Did not remove contacts. 4 8 25 - This am when EE awoke had a gunky eye and redness around eye.
