Before the incident
Aligning rod and cylinder.
What happened
Employee #1 cracked open the valve and the cylinder began to extend while employee #2 guided the rod end into the clevis. The rod den had gotten stuck prior to holes aligning. Employee #1 cut off the valve and Employee #2 tapped the rod end with a hammer in an attempt to free up rod end within the clevis. When the cylinder released the lower portion of the cylinder housing struck Employee #1 left arm just above the elbow.
