What happened
Employee was returning from the bathroom when he realized a forklift was parked behind his walkie rider. The employee attempted to maneuver his walkie in reverse around the forklift when his foot was caught in between the platform of the walkie rider and warehouse safety bollard. The employee stated that his foot wasnt entirely placed on the walkies platform.
