Changing Die Cartridge

At [REDACTED] maintenance was working to remove the die from Pellet Mill 2 for replacement. [REDACTED] was working an impact wrench behind the die while [REDACTED] was using a sledgehammer to vibrate the die to help in its removal as it was firmly stuck. When the hammer made contact with the die a piece of metal chipped off and hit [REDACTED] in the leg. We looked at it and observed an approximately 1 inch laceration on his inner thigh slightly above knee level. We administered basic first aid sanitizing the area and wrapping it with some gauze. There was no blood spillage and materials were disposed of in the proper receptacle. [REDACTED] was able to walk normally and didn 't mention any serious pain or discomfort. I made contact with [REDACTED] and [REDACTED] while conducting the accident investigation report. While I was talking with [REDACTED] he said he would like to get it checked out at the hospital in case it needs stitches. I called our office manager [REDACTED] she on her way
