Before the incident
Two employees were engaged in transporting a loaded product cart with one employee pushing and the other in front pulling (see Photo Addendum 1) As they moved the product cart into a cooler it caught the lip of a drain (see Photo Addendum 2) and began to tip forward The employee who was pulling attempted to catch the product cart but got his hand caught between the lip of the cart and the floor
