Mechanic was assembling a PVC pipe

The compounder was working in Sector 1 at the bottom third hole of the tank. After adding both liquid and powder ingredients down the chute and completing the mixing process he needed to verify that the materials had fully dissolved before proceeding. Access to the back of the tank was restricted because the mixer prevented him from using the ladder so he attempted to carefully maneuver around the tank and adjacent piping to reach the inspection point. As he squeezed and shifted his position in the narrow space he stepped onto an area of the floor contaminated with an oily substance. The surface was extremely slick causing him to lose his footing. He slipped and fell backward landing on his back while wearing his PAPR backpack with his right knee bent and caught underneath him.
