Before the incident
the employee was moving a tank from the mohawk area to the large wash area. Due to the crowded conditions he was maneuvering the tank using an old pallet jack alternating between pushing and pulling to make a u-turn.
What happened
while repositioning one of the pallet jack wheels became stuck in a floor drain causing the jack to stop suddenly. The dispenser slipped on the wet floor and fell forward striking his left inner knee and upper thigh on the pallet jack.
