Before the incident
Employee was supplied with inspected working PPE Employee had training on the correct use of equipment. Employee was attempting to move an object under load using an incorrect procedure. Employee did use a strap for use with a crane but did not use the crane nor secure the strap properly.
What happened
Employee attached strap to the front corner of a block of wood failing to ensure that the strap was securely in-place. Employee then attempted to move the load off of the block of wood by hand as opposed to using the crane. The strap slipped up the side of the block of wood and released causing the employee to tumble and injure their ribs.
