[REDACTED] [REDACTED] was running the lathe machine in the die shop which carves dies with metal wires. He stepped away from the machine for a moment to attend to another task.

Upon return to the machine [REDACTED] felt chips flying away from the machine and onto his right foot which cut through his work boots. He immediately pressed the emergency breaks to turn off the machine.
