Two production leads were ejecting a bail at the bottle crusher using standard procedures including raising a pallet with a forklift to accept the bale.

As one lead used the pendant controls to push the bale forward the other stood beside the forklift. When the bale was halfway out a baling wire snapped and impaled the lead’s upper left arm
