Before the incident

Employee was walking down the ramp with a small cleaning cart. Another employee that should not have been on a forklift came down the ramp. the forklift began to slide and gained speed. [REDACTED] (sanitation supervisor) and [REDACTED] (safety manager) shouted at them and the employee walking down the ramp began to run. Neat the bottom of the ramp the back side of the forklift that spun around impacted the employee on foot. The employee was thrown and spun 10 feet.

What happened

There was a flood in the basement in the [REDACTED]. Employees were cleaning it up all day. It was closed to forklift traffic all day even though we needed items from the basement. There were two floor fans plugged in at the top of the ramp and one at the bottom of the ramp blowing onto the ramp and drying the floor. [REDACTED] minutes prior to the incident the safety manager had made sure the caution barrier was in place prior to her going down the ramp. The employee on the forklift said the tape was not there. The fans were still there. The employee on the forklift was sent a text from his supervisor XX see me when you get in The employee replied I 'm about to meet you in the basement right now

Injury or illness

The forklift hit the employees back. The EMT 's did not see any outward blood or broken bones but said he would need to be checked for internal injuries. Employee was in pain but conscious and able to move his limbs.
