What happened
Die Maint EE was trying to hold a spool (in the middle of two die blocks) with a wrench and use an impact gun to tighten the bolt on top. The bolt was cross threaded and was too tight to move in the spool as a result the wrench that was on the spool spun around and caught the EEs finger between the blocks. EE has a fractured R index finger and off work until follow up with orthopedic.
