What happened
An employee was in the process of cleaning saw dust off of a notching machine using compressed air. To do this the employee disconnected an airline from the machine and started blowing away dust. When the air is disconnected from the machine a large metal part that is pneumatically powered can then move freely back and forth along rails. At one point while cleaning the employee leaned down to get under the notcher placing their hand on the table in the rails of the sliding potion of the machine.
